• Please fill it out, sign it and bring it with you on the first day of camp.   
  
• There  is a space in Part B to sign if you would like to give us permission to  administer over the counter medications, like Tylenol, Benadryl or  Pepto.  
  
• This form does not need a doctor's signature
  
•  If your youth does not have medical form on file, they will need to leave the camp.  No exceptions!
